F/T Case Manager for the Partial Hospitalization Program

DESCRIPTION OF DUTIES:

This is a professional position responsible for managing and coordinating all facets of a child's treatment in the Partial Hospitalization Program.  Works closely with the assigned therapist and clinical care staff to complete treatment plans and ensure on-going quality of treatment. 

Hours

M-F 8:30am-4:00pm

37.50 hour work week

QUALIFICATIONS:

Education/Training/License/Certification:

Minimum of a B.A. in social services or related field. 

Work Experience:

Minimum of two to four years of related experience is required.

Minimum Entry Pay Rate

$18.54

*Entry rate may be adjusted for relevant job experience.
